Indiana's innovative Pathways for Aging program empowers seniors with choice and control over their long-term care.  A crucial aspect of this program is the network of Managed Care Entities (MCEs) that play a central role in coordinating care and services. But with multiple MCEs (Anthem, Humana, and United Healthcare) available, how do you choose the one that best suits your needs?


Understanding MCEs:

Think of MCEs as your partners in navigating Pathways for Aging. They'll:

  • Develop a personalized care plan:  Working with you and your doctor, MCEs create a plan that addresses your specific needs and preferences.

  • Coordinate services: MCEs ensure seamless access to a range of in-home and community-based services, including personal care assistance, transportation, and social supports.

  • Monitor your health: MCEs keep track of your well-being and make adjustments to your care plan as needed.


Choosing the Right MCE:

Here are some key factors to consider when selecting an MCE:

The Indiana Family and Social Services Administration (FSSA) provides valuable resources to help you compare MCEs. Here's how to get started:

  • Visit the FSSA website: Indiana Pathways for Aging provides information about participating MCEs, including contact details and potentially, service area maps.

  • Contact your local FSSA office:  They can answer your questions about MCEs and potentially connect you with a representative for further guidance.

Pathways for Aging starts on July 1, 2024. Please consult with your current plan coordinator/manager for help during this transition if you have questions. You should be receiving information via mail and phone from your current provider.
